- Must possess valid Illinois licensure or be eligible. Must obtain CPR certification.

- Graduate of an approved APTA school of physical therapy.

The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and ability to adjust focus.
PRIMARY JOB DUTIES
- Evaluate and manage dysfunction to enhance physical and functional abilities.
- Perform examinations that include interpreting physician referral, taking a patient history, conducting a systems review, and performing tests and measures to identify potential and existing problems as well as determining appropriateness for rehabilitative intervention.
- Based on clinical judgment, treatment diagnoses, and prognoses, establish both individualized short- and long-term goals for an episode of care that meet the requirements of the referral and the needs of the patient.
- Utilize interventions that are supported by evidence-based practice to be effective for achieving patient goals as established in the initial evaluation
- Modify interventions as necessary in an effort to promote efficient and effective treatment in order to achieve anticipated goals and expected outcomes.
- Effectively manage a patient’s episode of care via appropriate utilization and clinical supervision of ancillary staff, effective communication with the referring provider, and providing efficient and effective interventions.
- Attend required meetings and participates in committees as requested; participates in professional development activities and maintains professional affiliations.
- Performs other duties as assigned.
