PEER RECOVERY WORKER
Responsible for delivering non-clinical activities that engage, educate, and offer peer support to individuals, their family members, and caregivers to successfully connect them to culturally relevant health services, including prevention, timely treatment, recovery management, and follow-up. The health navigator assesses healthcare needs and identifies community resources; serves as a patient advocate and assists in theidentification and improvement of service delivery.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provide peer health navigation to participants through multiple and ongoing recovery sessions.
- Create and build effective working relationships with patients, multidisciplinary care providers, and community partners that are characterized by active, cooperative treatment planning, problem-solving, counseling, and referral provision.
- Provide in-depth, ongoing risk reduction counseling that addresses specified behavioral objectives while working to eliminate barriers to change.
- Maintain accurate, current, and ongoing treatment plans and progress notes on each participant.
- Maintain an accurate and up-to-date culturally appropriate referral list.
- Act as participant advocate within internal and external support service systems.
- Provide resources for transportation to participants to appointments, as needed.
- Create and maintain comprehensive participant files.
- Track and locate missing participants.
- Perform other duties as assigned.
QUALIFICATIONS:
- Good understanding of the health resources available in their communities to refer their patients to the best resources available to them.
- Must be skilled in communicating with and facilitating dialogue between health care professionals, individuals and their families.
- Must be able to demonstrate active listening, empathy, relationship building, and interpersonal skills.
- Ability and willingness to disclose lived experience of mental and/or substance use disorders and recovery with peers, professionals, and the public.
- Must demonstrate knowledge and ability to integrate health information, about prevention/management of recovery and the health system, into the culture and language of the community. Must have knowledge of communities they serve.
- Ability to work one-on-one with individuals from diverse socio-economic backgrounds. Exceptional written, verbal and computer skills, including organizational and problem-solving skills, with the ability to work independently and collaboratively; a thorough understanding of confidentiality and the legal consequences of failure to abide as well as understanding and awareness of professional boundaries.
- Ability to research, access and disseminate community referrals.
- An aptitude and preference for detail attentiveness, organization, and order.
Job Type: Full-time
Pay: $15.00 - $18.65 per hour
