SUMMARY:
This Pediatric Health Case Worker (PHCW) shall assist the Pediatric Care Team and Maternal, and Child Health Unit (i.e. MCH Unit Manager, CPSP MA and Pediatric Providers) in providing case management of pediatric patients. This position shall ensure that pediatric patients attend their regular well-child medical visit as highlighted in the Bright Future Periodicity Schedule (PERIODICITY SCHEDULE 2021.indd (aap.org)). The responsibilities of the Pediatric Health Case Worker shall include administration and handing of developmental screening using Ages and Stages Questionnaires, 3rd Edition (ASQ-3). In addition, case management of referral orders for hearing screen and follow-ups. Conduct lead lab test schedule follow ups for missed, cancelled or no-showed appointments at APHCV or at Quest Diagnostics.

DUTIES AND RESPONSIBILITIES:
- Administration and Handling of Developmental Screening
- Provide support to the Pediatric Care Team by setting up the appointments with the patients to administer the Ages and Stages, 3rd Edition (ASQ-3) developmental screening questionnaires during 9 months, 18 months and 30 months.
- Track and case manage referral orders for developmental screen findings submitted for the pediatric patients and conduct follow-ups on appointments with Regional Centers or Specialist (i.e. Childrens Hospital LA).
- Submit all referral orders documentations and appointment intake forms to Regional Centers and Specialist.
- Conduct calls and communicate the referral orders and appointment schedules with the patients. Do follow-ups and reschedule missed, cancelled or no showed appointments of the patients and document all communications in the patients medical charts.
- Complete referral order loops by following up on diagnostic testing results, consultation reports or progress notes.
- Other tasks which the MCH Manager may assign.
- Case Management of Hearing Screening
- Ensure that hearing test result is in the newborns hospitalization record received from the delivering hospitals. Obtain any missing medical records requested from the delivering hospitals if needed (i.e. Hollywood Presbyterian or Adventist Health Glendale etc.).
- Ensure that hearing tests orders are submitted for all pediatric patients starting at age 3 and then for pediatric patients ages 4, 5, 6, 8 and 10.
- Track and case manage all pediatric patients referred for audiology by submitting referral orders documentations and appointment requests to the Specialist.
- Communicate the referral orders and appointment schedules with the pediatric patients. Do follow-ups and reschedule missed, cancelled or no showed appointments of the patients and document all communications in the patients medical charts.
- Ensure that the audiology referrals orders are complete and close the referral loop by obtaining the consultant reports and progress notes.
- Ensure complete documentation of all calls or communications in the patients electronic medical records.
Case Management and Tracking of Lead Blood Testing Orders
- Ensure that blood lead testing is ordered for all pediatric patients at age 12months and 24 months.
- Track all blood lead testing orders and assist in scheduling the blood draw at Quest Diagnostics if unable to perform the blood draw at the clinic on the time of the appointment.
- Assist the MCH Unit Manager and/or the MCH Unit MA in managing those patients/IPA members on the gap list (gap in care). Call and schedule blood lab draw at the clinic or at Quest Diagnostics.
- Conduct calls and document all communications in the patients electronic medical records.
- Conduct Health Education and Promotions for Certain Health Conditions
- Provide teaching in managing certain health conditions for instances, importance of lead testing, developmental screen and anticipatory guidelines to prevent risks and for the pediatric patients to attain optimal health and development.
- Conduct health education outreach/inreach using APHCVs text messaging and group messaging broadcast platform and patients portal.
- Manage the regular weekly and monthly scheduled outreach/in-reach text messaging for specific pediatric target groups.
- Provide specific health education materials to the pediatric health care team and ensure these materials are readily available in the exam rooms.
- Promote Effective and Efficient Patient Case Coordination
- Tracked completion of appointments per Bright Futures Periodicity schedules recommendation. Facilitate in scheduling appointments if patients lacking the appointments.
- Assist other clinic staff in providing good efficient case management and follow-ups.
- Maintain proper and accurate documentations and logs.
- Assist in providing translation and interpretation services as needed
- Facilitate patient flow by working effectively and efficiently.
- Maintain open communication with other clinic staff.
- Maintain professional demeanor at all times with patients, caregivers, clinicians, and staff.
- Participate in data collection and entry as assigned
- Provide administrative assistance to the MCH Unit Manager or the Pediatric Providers.

- Ensure that patients receive accurate language appropriate education, information, and instructions as ordered by clinicians through the aid of language interpreter or SSL.
- Review and pass (80%) of Pediatric Health knowledge competency.
- At least 18 years of age
- High school diploma and GED or minimum of one year experience working in a primary care setting
- Regular predictable and timely attendance during clinic hours
- Proficient in PC/MAC with working knowledge of Microsoft Office applications.
- Experience with intranet and internet.
- Computer literacy in English and targeted language.
- Basic knowledge of pediatric care and health in general.
- Strong communication skills.
- Ability to work efficiently, accurately, and with minimum supervision.
- Ability to follow directions and follow up on recommendations from supervisor.
- Ability to work with people of diverse cultural, educational, socio-economic, and linguistic backgrounds.
- Knowledge of health care system in the United States.
