Patient Care Coordinator Must be a RN

⦁ Ensures compliance with nursing care standards established by commissions, and federal and state regulatory agencies, and adherence to Agency objectives and policies pertaining to nursing administration.
⦁ Manages resources, facilitates concepts and approaches, reviews projects' progress and results; makes management decisions.
⦁ Participates in the selection, hiring and termination process of staff
⦁ Determines workload and delegates assignments, training, monitoring and evaluating performance, and initiating corrective or disciplinary actions in accordance with Agency policy and in a timely manner.
⦁ Responsible for meeting the requirements for contracted staff.
⦁ Responsible for the scheduling and coordination of patient care staff. .
⦁ Acts as a resource to provide information or determine the most effective way of meeting the needs of staff, patients, or customers.
⦁ Reviews and assures accuracy of invoices related to areas of responsibilities, in a timely manner.
⦁ Responsible for supplies and PAR levels.
⦁ Participates in nursing leadership on call rotation schedule related to clinical issues.
⦁ Monitors clinical documentation, assuring accuracy, completeness, and compliance with licensing regulations, certification standards, and Agency's policies and procedures, including but not limited to the Comprehensive Assessment, Patient Medications,
and Patient/Family Plan of Care
⦁ Participates in the process for assigned areas during any federal and /or state survey process or compliance activity as needed.
⦁ Acts as a contact for information relating to clinically assigned areas for both staff and external sources
⦁ Ensures appropriate scheduling for areas of responsibility and monitors and manages appropriate staffing levels in accordance with Agency productivity staffing standard.
⦁ Participates in the pre-assessment and admissions process.
⦁ Participates in and is responsible for quality assurance, improvement and utilization review activities.
⦁ Assists in coordination of IDG supervisory meetings.
⦁ Responsible for meeting the requirements of Hos/MCare/MCaid certifications and recertifications
⦁ Monitors and ensures appropriate utilization of patient related DME, supplies and biomedicals.
⦁ Develops and presents education programs for CHC and outside agencies.
⦁ Completes documentation related to any direct/indirect patient care contact.
⦁ Functions in the role of Case Manager as needed based on the current census or staffing vacancy.
⦁ Participates in patient care by functioning in the role of Triage, home visits, and emergency visits.
⦁ Coordinates and prepares for the IDT meetings.
⦁ Accountable for Agency relationships in the LaPorte area including hospitals, ECFs, group homes, and assisted living facilities.
⦁ Attends ECF care plan conferences.
⦁ Accountable for state and federal regulations relating to hospice and home health.
⦁ Ensures appropriate utilization of levels of care for each home health patient.
⦁ Assists with maintaining quality assurance with implementation of protocols, procedures, etc.
⦁ Participates in the development of policies and procedures as clinical practice changes.
⦁ Ensures and complies with HIPAA regulations.
⦁ Performs other duties that may be assigned.

QUALIFICATIONS:
⦁ Maintains current R.N. licensure required in Indiana.
⦁ Previous supervisory experience required.
⦁ Previous nursing experience in home health/hospice preferred
