Post-Acute Notre Dame Hospice is seeking a Patient Care Coordinator for the Northshore!
$18-$24 per hour based on experience!

QUALIFICATIONS:
Educational/Degree: High School Diploma or equivalent
Knowledge/Skills/Ability: Ability to work independently and in groups depending on the task. Ability to remain professional when dealing with patients, families, management, and all internal and external customers. Acceptance of and adaptability to different social, racial, cultural and religious modes.
Experience: Minimum two (2) years of experience in a healthcare office setting preferred.
JOB DUTIES:
Routine Tasks
- Listen to report line
- Morning Stand Up Meetings
- Send out morning page to team
- Update Discipline Load
- Update Suncoast or designated EMR (Level of care, Addresses, Date of Death, Etc)
- Update Coroner’s report
- Update Census
- Update Team Agenda daily
Admissions
- Complete Initial Chart Checklist, or designated forms, as needed
- Complete Suncoast or designated EMR entries, as needed
- Print face sheets and distribute to patient team and on-call team
- Fax off release for additional H&P, as necessary
Documentation
- Follow up on outstanding Initial Chart Checklists, or designated forms, as needed
- Follow up on any outstanding signatures for certifications
- Maintains filing of patient’s charts and performs basic chart audits
Payroll & Scheduling
- Complete schedule for CNA with Patient Care Manager oversight
- Verify time sheets and end enter into Attendance Enterprise, or designated systems
- Notify Patient Care Manager of outstanding timesheets via email
- Notify Patient Care Manager of potential overtime via hours summary report weekly
- Complete payroll and submit to Patient Care Manager every Monday
- Complete Visit Tracker and report any anomalies to Patient Care Manager
Stewardship
- Notify Nurse Practitioner of upcoming Face to Face visits, or other clinical requirements
- Pre-bill review bi-weekly with Patient Care Manager and other staff, as needed
- Complete any necessary billing requirements (Medicaid, Private Insurance, Private Pay, Veterans Benefits, etc)
Administrative Duties and Staff Development
- Handling First Tier Calls (Supply requests, contacting staff members for family (non-critical), death certificates)
- Complete HIS on admissions/deaths/discharges within required time frames
- Serves as back up to the front desk and admissions, as needed
- Assist with Orientation, as needed
The Patient Care Coordinator serves as resource person and role mode
