Summary:

A Patient Care Coordinator is a key member of the patient care team, ensuring that patients get the best care possible. A Patient Care Coordinator listens to patients and their families to ensure needs are being addressed, educating patients on their medical procedures or conditions and acting as a liaison between the patient and healthcare provider. The Patient Care Coordinator performs clinical tasks and procedures to support the delivery of care and promote efficient use of the physician's time.

Essential Duties & Responsibilities include but are not limited to, the following:

Work as a team player to ensure each patient receives the best service possible

Provide exceptional customer service- including providing compassionate, empathetic care in a professional manner

Answer the telephone with a pleasant demeanor, always identifying self and location

Use professional communication skills verbal and written with all members of the clinical staff and external members

Assist care team in providing highest level of care by coordinating with referring doctors and community resources,

Screen physician s schedule one week in advance-

Obtain medical records for all new patients prior to visit- particularly ENT and Allergy referrals

Fill out disability forms

Send out medical records

Authorize medical supplies i,e, tracheostomy supplies, out of network medication authorization, BOTOX authorization

Maintain open lines of communication between patients, families and healthcare providers- including EPIC my chart messaging- Regularly communicates with clinical staff regarding patient health issues and complications

Returns phone calls or answers questions email, epic, etc, from concerned patients

Calls postop Day 1 patients to see how they are doing

Review urgent lab, imaging or pathology results

Daytime ER consult triaging

Confidentially manage patient information

Understands and adheres to Rutgers compliance standards as they appear in Rutgers and RWJBH Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy

Scanning medical records into EPIC

Schedule patient visits for preoperative evaluation, postoperative evaluation and post imaging evaluation

Place preoperative Pre Admission Testing orders

Answer pre-visit questions, help with educating patients re: treatment options, help arrange referrals to specialists and ancillary services

Answer routine postoperative requests

EDUCATIONAL REQUIREMENTS:
Bachelor s Degree in a healthcare-related field such as: Nursing, Surgical Technology, Athletic Training, Health Informatics, or a related field. Equivalent education, experience, and/or training may be substituted for the degree requirements.

QUALIFICATIONS AND SKILLS:
1. One (1) year of experience working in a physician group practice is preferred.
2. Must have significant knowledge of computer applications including Microsoft Word, Excel, practice management software, PowerPoint, as well as a web-based software
3. Ability to perform multiple and diverse tasks simultaneously required.
4. Ability to prioritize work flow in a fast-paced environment
