Schedule: Monday - Friday 7:30am - 4:30pm

GENERAL DESCRIPTION SUMMARY

The Patient Care Coordinator will support daily operations of a Hospice Care in the home care and facility setting. This includes daily administrative functions, ensuring great patient and family experience, managing CNA schedules and operations for providers, managing the plan of care needs for the CNA and communicating changes to the patients/families so all are informed on a timely basis of any changes. The Patient Care Coordinator will work with the clinical team and executive leadership to ensure high quality of daily operations, supplies and equipment, and overall support for the office team.
Essential Duties & Responsibilities - Within the philosophy, objectives and policies of Capital Caring Health, carries out the following essential functions:

Quality Provision of Services:
1. Oversees the daily office operations.
2. Responds to incoming phone calls, screen and triage calls as appropriate.
3. Reaches out to patients and/or caregivers via telephone for routine scheduling and changes in schedules.
4. Maintains patient information in Electronic Medical Record (EMR).
5. Monitors and manages encounter notifications from family and clinical team.
6. Maintains efficient and effective CNA schedules based on the needs of the patient.
7. Supports team members to ensure documentation is complete, accurate, and timely.
8. Assists with orientation of new staff and ensure a successful onboarding process.
9. Assists clinicians in ordering DME, completing EDRS, referrals, and other patient support activities as directed.
10. Provides administrative/clinical support as back up to Office Manager and/or Receptionist as needed.
11. Performs other duties as assigned.

Organizational Compliance:
1. Assists with development of policies and procedures and enhance processes for patient/family satisfaction.
2. Ensures administrative practices meet regulatory guidelines, including state, federal and local requirements.
3. Ensures all HealthStream educational modules are completed in a timely manner for CNAs.

People/Communications:
1. Provides high level of satisfaction for all stakeholders, internally and externally.
2. Researches and follows up with appropriate individuals to resolve issues/concerns.
3. May act as a liaison between providers and patients/family members.

Financial Stewardship:
1. Achieves expected productivity standard as defined by the organization and in alignment with best practices/the Capital Caring Health model.
2. Uses good stewardship in provision of supplies and services as needed for implementation of care
