Aurora Mental Health Center (AuMHC) is seeking a Patient Care Coordinator to provide comprehensive patient care coordination to severely persistent mentally ill adults and adults with intellectual and developmental disabilities. The care consists of medical case management and care coordination. We are looking for a candidate who is a self-starter, embodies integrity, humility, and has a personal sense of responsibility and passion for underserved populations. About the team.

The Patient Care Coordinator will be a part of the Physician Operations team and support our Adult Intensive Services. Adult Intensive Services specializes and supports adults who have intellectual and developmental disabilities, co-occurring mental illness, and severe and persistent mental illness.


$2,000 Sign On Bonus Available!
*Not eligible for internal and PRN candidates.

Based on full time (40 hours/week). Prorate if less than 40 hours/week. Paid in two installments: 50% on first paycheck, 50% on paycheck following 6-month anniversary.


Salary for this role.

  • $18.89 - $28.33 per hour (based on experience and company equity)


Responsibilities:

  • Foster a culture of patient focused care, understanding that the quality of the relationship is a key factor in healing.
  • Actively collaborate and communicate with staff across the agency to provide outstanding patient care.
  • If authorized and appropriate, cultivate positive relationships with each patient's outside providers to enhance care continuity and integration.
  • Promote, encourage and model the practice of continuous improvement with a focus on patient experience, access, safety and treatment effectiveness.
  • Help clients evaluate strengths and symptoms, and facilitate in clients setting their own goals and plan for appropriate services.
  • Offer support, assistance, consultation and education to families, friends, landlords, employers, community agencies and others who come into contact with clients (case management) in order to maximize benefits and minimize problems associated with the presence of these persons in the community.
  • Reach out to eligible clients, inform them of and educate them about available services at the organization and in the community.
  • Understand and comply with organizational goals around quality, compliance, standards, and policies.
  • Complete required education and training in a timely manner.
  • Accurately and completely document patient care (CCARS, COMPASS etc.)
  • Provide information to credentialing, payroll, etc. in a timely manner.

Requirements:

  • Bachelor's degree.
  • Preferred: Master's degree.
  • Preferred: three years' of relevant clinical work experience.
  • Advanced computer skills, including use of electronic health record, MS Office products and the ability to learn new tools as needed.
  • Ability to exercise independent judgment and discretion.
  • Excellent written and oral communication skills.
  • Strong customer service and interpersonal skills.
  • Ability to organize and prioritize workload.
  • Detail-oriented with ability to multi-task.


Required Vaccinations and TB Test.

AuMHC has prioritized the health and safety of our clients and staff and requires all employees to be fully vaccinated against Covid-19, receive an annual influenza (flu) shot, and receive negative TB test results prior to their first day of hire. Proof of both vaccinations and negative TB test will be required upon your first day of employment. Medical and religious exemptions may be requested for either COVID 19 vaccine or annual flu shot.
