Panel Manager*

Location:* West Salem Clinic; 1233 Edgewater NW, Salem OR. 97304
Job Status: Full-time, Monday-Friday

We are hiring a knowledgeable Panel Manager to collaborate with a multidisciplinary team to improve health outcomes of patients using population health strategies.

YOUR ROLE:*
The Panel Manager in collaboration with our clinic leadership team, will identify population management priorities and develop corresponding interventions and campaigns. Using a continuous process improvement approach, you will strive to enhance quality of care and chronic disease management systems. This position facilitates our patients' access to appropriate care services and follow-up care related to population health management.

SPECIFIC DUTIES:

  • Participate in quality improvement meetings and events as assigned.
  • Perform out-reach and in-reach population management using calls, mailings or any type of media required.
  • Schedule patients for preventive care needs and follow up to ensure completion of services.
  • Assist in rearranging providers’ schedules when called away from the clinic, is ill, or has a change of schedule.

QUALIFICATIONS:

  • High School Diploma or General Education Development (GED).
  • Ability to interact effectively and courteously with patients and staff.
  • Proficient in typing, familiarity with computers and legible handwriting.
  • Ability to work in a fast-paced environment
  • Bilingual English/Spanish preferred.
