Major Responsibilities:
Coordinates interdisciplinary team services, including team development and education.
Initiates care conferences and conducts patient and family interviews.
Develops a relationship with patient and primary physician to facilitate the palliative program. Acts as a patient advocate throughout process.
Collaborates with interdisciplinary team members in the development, implementation and review of clinical protocols.
Collaborates and assists with financial resources, advance directives, psycho/social issues, end of life support, references to hospices, Aurora at Home, and community resources.
Coordinates and actively participates in the education of medical center staff on palliative program.
Assists Administrative Director of Nursing Services with the development of the budgets (capital, personnel and operating) and monitors the financial and operational impact of the program.
Develops and implements quality measures for successful program outcomes. Collaborates with VLCC, medical staff, and Aurora at Home to ensure quality of the program.
Able to assist all parties to be successful with discharge planning decisions in the palliative program.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:
Social Worker license issued by the state of Wisconsin.

Education Required:
Master's Degree (or equivalent knowledge) in Social Work.

Experience Required:
Typically requires 2 years of experience in hospital social work that includes experiences in oncology, internal medicine, geriatrics, critical care, or hospice.

Knowledge, Skills & Abilities Required:
Excellent interpersonal and communication (written and oral) skills.
Intermediate computer skills with electronic patient record systems, Microsoft office products, and electronic mail.
Ability to work in a fast paced environment with complex patients and family situations
Ability to be flexible, multitask and collaborate with others

Physical Requirements and Working Conditions:
Must be able to stand, walk, bend, stoop, and twist continuously through the workday.
Must have functional speech, vision, touch, and hearing.
Will be exposed to the following hazards on a continuous basis; mechanical, electrical, chemical, and radiation as well as blood and body fluids; therefore, protective clothing (i.e., goggles, lead apron, gloves, gowns, etc.) must be worn as necessary.
Exposed to a normal office environment.
Operates all equipment necessary to perform the job.

This role will have 0.5 accountability to palliative care (working alongside palliative care NP) and 0.5 accountability to inpatient social work helping with discharge planning, consults and more. Monday-Friday 8 hour days Will include weekend rotation (1 every 6 weeks and on call (1 week annually).
This is a small department (10 total RN CMs and SWs), works very well together, works closely with providers, supportive leadership
