PACE of the Southern Piedmont, a not-for-profit, community-based organization committed to the wellness of seniors, is currently seeking an experienced Registered Nurse to join our dynamic team.

Registered Nurse duties to include but not limited to:

  • Assists with the coordination of care for PACE of the Southern Piedmont participant’s that reside in Long Term Care (LTC) and Assisted Living (ALF) facilities.
  • Conducts chart audits with contracting LTC and ALF vendors.
  • Makes routine visits with LTC and ALF participants.
  • Performs wound care, skin care and other treatments as ordered by the Primary Care Provider (PCP) and documents in the EMR and logs as appropriate.
  • Documents participant care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems.
  • Works with Clinic Technician and PCP along with the IDT to assess participants with holistic needs in the clinic setting. May provide End of Life Care and visits as requested.
  • Completes in-home aid re-assessments and supervisory visits as requested by the HCA Supervisor. Provides feedback to the IDT and the HCA on results of those visits to support participants’ plans of care.
  • Participates in the coordination of 24-hour care delivery while participants are at home, in the hospital, or in a nursing facility, including medication reconciliations. Communicates participant changes to team members in a timely and appropriate manner.
  • Takes on-call as scheduled on a rotating basis and documents those calls promptly.
  • Facilitates Corrective Action Plans (CAPs) as directed by the Medical Director and/or Leadership Team when opportunities for improvement are identified. Process includes identification of global problems, development of improvement plans, and implementation of plans to then evaluate their effectiveness. Facilitates CAPs from contracted providers in collaboration with the Director of Finance and Administration.
  • Assists in interface between consumers and program management for quality related complaints. Ensures compliance with Federal, State, and Local laws and regulations.

Required Experience: Has practiced nursing in the last 3 years; with at least 2 years of RN experience including 2 years of experience working with the frail elderly population.
