Are you an experienced RN or LPN looking for an exciting and rewarding new opportunity as part of a dynamic team? Fair Oaks Health Care Center, located in Crystal Lake, IL is looking a reliable experienced full- and part-time (12-hour shifts/7pm-7am) overnight LPN/RN with extra shifts available to pick-up if you choose. Fair Oaks has a 5-Star rating with low nurse to patient ratios, exceptional teamwork and strong management support. The ideal candidate will have excellent communication and organizational skills while being passionate about caring for our residents.

Job responsibilities include, but are not limited to:

• completes assignments/assessments in a timely manner ensuring safe care of residents
• document results of care within facility policies, procedures and scope of practice
• implements holistic plan of care
• demonstrates time management and task prioritization previous long-term care experience preferred

QUALIFICATIONS REQUIRED:

• must have active and current nursing license in the state of Illinois
• must be able to perform physical requirements of the job
• must successfully pass background and drug screen

QUALIFICATIONS PREFERRED:

• previous long-term care experience preferred
• CPR Certified'

Job Types: Full-time, Part-time

Pay: $26.00 - $38.00 per hour
