The Oncology Nurse Navigator position serves oncology adult and geriatric patient populations that are diagnosed with cancer and/or will be treated within Cone Health. The position acts as the primary resource for patients and families for all information and education. May be responsible for planning and managing the weekly oncology site specific conference and multidisciplinary clinic or coordination of patients in active treatment to cancer survivorship. Works collaboratively with all physician groups, their staff and navigators to ensure patient care is of the highest quality. Through metric outcomes, works to continually improve quality patient care.

Assist and collaborate with the physician to appropriately identify patients for clinical trials being conducted at CHCC.

May assist in the selection of cases for presentation at the specialty cancer conference and coordinates the retrieval process for all pertinent records or coordinate and assist patients transitioning to life after cancer.

Collaborates with all healthcare providers to provide a seamless process for all patients.

Collaborates with the MDC care team to assess, plan, and evaluate healthcare pathways required to achieve effective resource management and quality outcomes.

Coordinates the efficient and effective operation of a Multi-Disciplinary Specialty clinic or Survivorship Care Plan visists by assisting with patient scheduling, case selection, education and navigation of the patient through the system.

Participates in quality improvement, research, and grant writing as well as help coordinate cancer screening services with community outreach.

Provides consultation and education regarding cancer diagnosis and screening to patients, families, healthcare providers, and the general public. This includes providing high risk assessments to patients and family members.

Serves as a leader in the development and maintenance of cancer detection and information services throughout the health system.

Serves as the initial contact for newly diagnosed patients or cancer survivors and provides resources to those patients in the underserved population.

EDUCATION:
Associates, Nursing, Required
Bachelors, Nursing, Preferred
Graduate of a School of Nursing. BSN required or with approval of the hiring manager, experienced RN's with an AD or Diploma with an active plan to obtain their BSN within four years from date of hire may be considered.

EXPERIENCE:
Required:
Minimum of 2 years of oncology or surgery experience as a nurse or equivalent combination of education and experience
Preferred:
Oncology Nurse or Nurse Navigator certification
