HOW YOU'LL MAKE A DIFFERENCE:
Care and compassion are at the heart of what you'll do as a Physical Therapist, Therapy Support. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based therapy care for patients as directed by the attending physician. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey. Home care provides context and real-world perspective about what will really help patients restore their health.
HOW YOU'LL WORK:
You will spend approximately 80% of your work week providing comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You'll be accountable for completing accurate documentation and remaining compliant with all legal rules and regulations. You will spend approximately 20% of your week participating as a member of the Therapy Program Support Team. During that time, you will be responsible for quality reviews, updating therapy programing, staff coaching with agency leadership involvement, staff education and supporting SASH program and protocols.
REQUIREMENTS:
Physical Therapist with a current license in the state of employment.
Minimum one-year experience as a PT in an acute care setting. Home care experience preferred.
Current employee and in good standings
Excellent documentation and clinician skills
Experience in electronic medical record use
Excellent oral and written communication skills
Proficiency in Microsoft Office (Outlook, Word, Excel and Access), training development and publishing tools, and Learning Management Systems. Strong ability to learn new systems.
Detail oriented with ability to manage multiple responsibilities and ability to prioritize responsibilities.
Experience in presenting educational content
Valid driver's license and auto insurance in your name as a driver
Capable of all physical demands
OTHER:
Therapy Program Support Team time is one day per week from 8 am to 5 pm, Paid at 7 Points
