Occupational Therapist (OT) position - will work in a combination position serving both our Home Health Services and our local School Districts. This is a full time 1.0 FTE day shift position.
The Home Health Occupational Therapist will do both evaluations and treatments of home care patients in their homes and provide services necessary for the patients. Will design, organize and conduct and/or supervise the performance of subordinate personnel of medically prescribed occupational therapy treatment programs and education. The Occupational Therapist evaluates, records and reports patient problems and progress, providing effective communication with other members of the rehabilitation team. Will develop new services/programs as needed and assess the need for such services. Will provide treatment and evaluation to local school districts for OT services for students. Will need to acquire a DPI licensure which facility will assist to obtain.
Candidates must be licensed in the State of Wisconsin as an Occupational Therapist and have had prior work experience as a treating therapist. Experience with EMR's is preferred. Therapists will design, conduct and supervise the therapy services for patients and report patient problems and progress with members of the home care services team.
This is a full time position and will generally involve 3 days per week in the Home Care Services area and two days in the Schools.
