Nursing Facility for Mental Health (NFMH) Olmstead Advocate

POSITION OVERVIEW:
The Nursing Facility Mental Health Case Manager provides targeted case management support for nursing home residents. The NFMH Case Manager is responsible for the assessment and collaborative development of client goals as identified by evidence based practices. In addition, the NFMH Case Manager is responsible for the coordination of services and assistance needed to meet identified goals, with the overall objective to shorten the average length of stay for residential clients residing in nursing homes.

ESSENTIAL POSITION RESPONSIBILITIES:

1. Provides services to meet program goals, maintaining a caseload of approximately 30 clients at a time.

2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage, and timecard entries.

3. Maintains accurate and timely documentation of service provision. Completes case notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.

4. Schedules and participates in the development of a Wellness Recovery Action Plan (WRAP) and Treatment Plan with the client, family/caregiver, and CMHC NFMH Liaison utilizing the Preadmission Screening and Resident Review (PASSR) and person-centered care.

5. Provides periodic review of treatment and WRAP plans to monitor the client’s progress toward goals.

6. Develops a crisis plan with client using the WRAP book and WRAP card.

7. Monitors status of clients and provides level of personal and other supports consistent with client status. Notifies CMHC NFMH Liaison and Director of change in client status.

8. Coordinates other services required to meet identified goals based on the needs of the clients and congruent with the client’s level of functioning.

9. Attends all department/individual meetings, agency meetings, and trainings as required.

10. Adheres to Evidence Based Practice guidelines for Strengths Based Case Management.

OTHER POSITION RESPONSIBILITIES:

1. Maintains acceptable overall attendance record. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.

2. Exhibits appropriate level of technical knowledge for the position.

3. Works well with a team, keeps others informed of information needed. Treats others with
respect, maintaining a spirit of cooperation.

4. Demonstrates the ability to access family needs.

5. Maintains effective and professional verbal and written interactions with peers, customers, supervisors, and other staff. Uses diplomacy and tact in dealing with difficult situations or people. Demonstrates effective listening skills. Is receptive to constructive feedback.

6. Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identifies what needs to be done and takes appropriate action.

7. Performs work conscientiously with a high degree of accuracy.

  • Completes assigned work, meets deadlines without reminders/follow-up from supervisor or
others.

  • Meets goals and objectives as mutually agreed upon during last performance review (if
applicable).

POSITION REQUIREMENTS:
Applicants must possess a Bachelor’s degree in a behavioral health field and have at least 3 years experience. Preference will be given to those who have experience in working with individuals who have a dual diagnosis of substance use and mental illness; or those who have experience working with NFMHs and/or CMHCs. Applicants must have a valid Kansas driver’s license and consistent to a background check.

PHYSICAL REQUIREMENTS:

  • Driving (for purposes of community mobility)
  • Typing/data entry, writing/note taking
  • Lifting/carrying up to thirty (30) pounds
  • Bending/Stooping/Climbing stairs
