We're growing! With this rapid growth, we're looking for RNs for our Home Health and Hospice across the Phoenix Metropolitan area! We have caseloads available Central, East and West. RN Case Managers make their own schedules seeing homebound patients per the collaborated physician led care plan. RN Case Manager (Hospice) will be responsible for:

• Providing nursing care to hospice patients: providing assistance, understanding and education to the family/patient in the home setting
• Regularly assesses and reassesses the nursing needs of the hospice patient including necessary treatments and palliation of symptoms
• Teaches, supervises and counsels the hospice patient and family members about providing care for the patient.
• Participates and provides leadership in the team for regular interdisciplinary group meetings for facilitation of problem solving sessions and delivery of team support
• Develops the patient/family plan and of care with regular review and revision

RN Case Manager (Home Health) will be responsible for:

• Completing an initial home health assessment of patient and family to determine needs
• Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by a physician
• Evaluating patient care needs and developing care plans based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions
• Administering medications and treatments as prescribed by the physician
• Counseling and teaching the patient and their family in meeting needs

Ideal RNs meet the following qualifications:

• AZ RN License (required)
• Minimum of 6 months hospice/home health nursing experience
• OR ICU/ED experience of 5+ years
• At least 3 years nursing experience (required)
• Knowledge of Consolo or AXXESS electronic medical record system (preferred)
• Ability to pass background check (required)
