The Community Service Plan (CSP) provides a roadmap for continuing the Division’s work to provide comprehensive outreach, engagement and treatment for community members living with HIV and/or at-risk of HIV. To address social determinants of health and mitigate risk for poor outcomes among vulnerable populations, the new CSP HIV team has embraced comprehensive care coordination strategies and practices multidisciplinary, team-based care including integrated behavioral health services, intense care coordination and primary care.
Mobile Medical Unit Driver - Comprehensive Wellness - WC
The Mobile Medical Unit Driver will transport a 40 foot Medical Mobile Unit to and from the parking area to all designated community locations. Sets up and breaks down for the MMU operations. Operates and drives the mobile unit medical unit. This role will also support the practice in an administrative capacity when not driving.
Preferred Criteria
- NYS CDL Class B
- Experience as a school bus driver, shuttle driver or other large vehicle in populated city environment
- Bilingual – Spanish and English
Required Criteria
- High school diploma or GED
- NYS CDL Class C with passenger endorsement in good standing
