For a youth and family therapist who comes at difficult problems in new ways, Nexus FACTS has a unique opportunity to be part of a new, one-of-a-kind treatment team. Nexus-FACTS seeks a Mental Health Therapist for our new, grant funded Youth Care Transition (YCT). The YCT is a new opportunity to address the needs of youth with high levels of mental health and behavioral challenges who have completed their treatment in a Psychiatric Residential Treatment Facility (PRTF) or the Child and Adolescent Behavioral Health hospital (CABHH) when a discharge placement has not been located. As part of a multidisciplinary team in the YCT program, the Therapist will provide high intensity, home based services with a strong system’s focus to develop and support community-based placements for these youth in the Metro area.
Primary responsibilities:
- Creates and drives a trauma-informed environment, establishing and maintaining a positive treatment culture and making necessary changes to ensure success
- Collaboration with youth and family to work with a team and develop to the Nexus Individual and Family Guidance Plans ensuring staff understand and utilize them in adherence to policy
- Engage in and have active involvement of the Family Collaborative Intake meetings
- Complete Child and Adolescent Needs and Strengths functional assessment in accordance with the TCOM (collaborative) principals
- Through the collaborative effort with family, youth and providers, develop and monitor the individual treatment plans
- Provide individualized, youth-focused, strength and evidenced-based family and individual therapy models to youth and families. Examples: Trauma Focused Cognitive Behavior Therapy and Eye Movement Desensitization and Reprocessing
- Participate in all youth related treatment team meetings, using family centered language and youth driven principals
- Be available on-call for consultation/treatment direction and/or decisions when appropriate
- Assess readiness for discharge in conjunction with the treatment team. Coordinate and recommend therapeutic services for success after discharge
- Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition
- Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process
- Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional manner according to policy and regulated statutes.
- Educate and facilitate trainings for staff on trauma informed practices, clinical diagnosis treatment processes, and other clinically relevant training material
- Participate in team meetings, program clinical team meetings, therapy clinical supervision sessions, and in-service as required
- Ensure confidentiality regarding mental health, medical issues, background treatment and comply with all HIPAA related requirements
Required Education and Licensure:
- Master’s degree in social work, psychology, or clinically related field
- Licensure preferred or on track with licensure within related clinical field (LGSW, LCSW, LICSW, LP or LMFT) per state board requirements
- Valid driver’s license required
Preferred Education and Experience:
- Experience working with at-risk youth and families facing mental health challenges.
Travel:
- Regular business travel to family homes, community locations, and referral sites required.

Specific vision abilities required by this job include close vision and ability to adjust focus, and maybe required to lift-up to 50 pounds unassisted and up to 100+ pounds with assistance. Standing and walking for extended
