Central Minnesota Mental Health Center is currently looking for a Mental Health Practitioner: Care Coordinator in our Elk River location. Duties may include but are not limited to:

• Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
• Assist with treatment planning process by coordinating treatment planning meetings and assisting in goal achievement.
• Assist clients in addressing barriers to accessing services, including transportation, insurance, finances, and legal issues.

The pay range is $19.0575-$22.8690. The starting salary for each application will be based on years of experience for the position.

• The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17.
• DHS Clearance and clear sexual exploitations screening.
• Valid Driver’s License and acceptable driving record.
• Must be 18 years of age.
