Responsibilities:
Position Overview:
The Medical Social Worker, Emergency Department/Critical Care Unit is responsible for performing social work assessments and interventions/discharge planning as needed for inpatients, outpatients, and emergency department patients. The position works to maximize social functioning and health management continuity of patients and families impacted by illness, and provides appropriate social service intervention as part of a multidisciplinary team in an effort to meet the total needs of the patient. Applies strengths-based and person centered philosophy of care, demonstrates practical application of social work core values. The functions include but are not limited to crisis intervention, patient/family intervention, high risk screening, grief counseling, referring for financial or other identified resource needs, arranging/facilitating family/patient representative meetings with the healthcare team, assisting in post-acute placement on complex discharges, and engagement of appropriate agencies or community resources when high risk patients are identified. The Medical Social Worker, Emergency Department/Critical Care Unit serves on hospital committees, and participates in developing training materials and presenting to hospital staff.
Qualifications:
Required:
  • Master’s Degree in Social Work or equivalent
  • One (1) year of clinical and/or medical social work experience
Preferred:
  • Licensed Clinical Social Worker (LCSW)
  • Previous experience as a social worker in an acute medical setting
  • Emergency Department and/or Hospital experience
  • Knowledge of contemporary behavioral and systems theories relevant to healthcare, end-of-life dynamics and interventions, grief and bereavement counseling, substance abuse identification and interventions, support for victims of abuse, neglect or violence
  • Knowledge of community or financial resources for underserved or vulnerable populations
  • Bilingual (English/Spanish)
