Hospice Social Worker
This is a Per Diem position.
Job Summary: The Social Workers (MSW) evaluates and assesses patient needs for social service assistance, planning and coordination of clinical programs and acts as a liaison between the patient, hospital, Agency, Physician, Insurance Plans and community,
Locations: Temecula, Fallbrook, Sun City, Murrieta, Menifee, Hemet, and LA county.
Essential Duties and Responsibilities:
Documents the full assessment and create a plan of care with accurate orders, diagnosis and projection of visits for all disciplines necessary to support each patient in EMR.
- Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.
- Documentation completed and synchronized/submitted within 24 hours.
- Documents patient status in an accurate, timely and concise manner by use of EMR in patient home at the time of visit.
- Acceptance and response to referrals from interdisciplinary health care team regarding clients who may require social services assistance.
- MUST BE PRESENT FOR IDG AT THE HOME OFFICE IN CLAREMONT EVERY OTHER WEDNESDAY.
Minimum Position Qualifications:
- Completes necessary education to attain master's degree in Social Work in state of California
- Basic Life Support certification
- California driver's license and proof of current auto liability insurance
- 1 year of social work experience
