LICENSED PRATICAL NURSE

Reports to: Unit Manager or Director of Nursing

Team: Nursing

Position Summary

Through utilization of the nursing process perform nursing duties within the scope of practice, under the direction of a registered nurse , the LPN works closely with the RN & Manager/Supervisor to ensure the best possible functions of the assigned unit. The LPN nurse will maintain the standards of Nursing Care and practices within their scope and the facility to meet the physical, psychosocial, and rehabilitative needs of the residents.

Essential Duties and Responsibilities

  • This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.
  • Observe resident's condition and response to treatment and take necessary and appropriate action as required.
  • Recognize and report significant resident condition changes and take appropriate nursing action.
  • Administer medications and treatments accurately according to facility policy and procedure.
  • Participate in observation and directing of nurse aide/CNA performance.
  • Transcribe physician's telephone and written orders accurately.
  • Document nursing care rendered, resident response to treatment, and other pertinent resident data in the medical record/PCC.
  • Attend facility in-services to remain current in nursing practice.
  • Practice acceptable infection control procedures, specifically hand washing, standard precautions isolation, aseptic, and sterile dressing techniques.
  • Adhere to all Department of Commerce, County and department safety policies and procedures
  • Makes rounds on units to observe, examine, interview residents, to evaluate staffing needs, to inspect care environment, and to evaluate staff interactions and clinical skill competency.
  • Responsible for ensuring adequate staffing on the units and makes changes as necessary, utilizing the resources available i.e. Smartlinx Scheduling application. Evaluates staffing patterns based on unit census, resident acuity, other resident needs. Adjusts staffing pattern accordingly.
  • Interacts with resident, family members, legal representatives, surrogates to answer questions, resolve concerns.
  • Conducts skills observations of nurse aides and Med Techs.
  • Actively participates in long term care survey process. Provides or offers supplemental information to ensure accurate survey results. Responds to surveyor questions. Records actions of survey team members and reports to DON at end of each survey day. Consults with DON about undertaking corrections while survey is in progress.
  • Completes required documentation of care and services delivered during shift including interventions and resident responses to interventions. Completes required documentation of special circumstances including accident/incident reports in compliance with State and Federal laws and regulations
  • Reports relevant information to DON including interdisciplinary team functioning and communication, regulatory compliance issues, resident risk factors, sudden changes in resident behavior, expressed dissatisfaction by resident or family, refusals of treatment, unexplained injuries, staff behavior (positive and negative), staffing needs, equipment and supply needs, opportunities to revise policies, procedures, improve quality.
  • Monitors staff for compliance with OSHA mandates on workplace safety including hazard communication and blood borne pathogens. In cases of workplace exposure, provides immediate first-aid and refers employee to appropriate facility staff member for follow-up.
  • Supervises performance of unit clerk and desk nurse in all essential functions of that position.
  • Reports all injuries to self or others to supervisor immediately.
  • Takes job actions with staff members under appropriate circumstances. Applies facility policies about progressive discipline correctly. Removed staff member from unit if necessary to avoid endangerment to health or safety of residents or other staff members. Reports all job actions taken to DON promptly.
  • Attends continuing professional education programs and in-service education. Applies information to job responsibilities.
  • Participates in fire and disaster drills. In the event of an emergency carries out assigned duties to assure resident safety
  • Supervises Certified Nursing Assistants, Med Techs, and/or Licensed Practical Nurses in the implementation of plans of care to assist residents to attain or maintain highest practicable physical,mental and psycho social well being.

Other Duties and Responsibilities

  • Carries out all duties in accordance with the Ridgewood Care Center mission.
  • Arrive and begin work on time and to report for duty as scheduled on all shifts, weekends, and holidays
  • Contact & communicate with Human Resourses for any absence/L.O.A if 2 or more days absent.
  • Contact Scheduler 2 or more hours prior to shift start if you can not arrive for your scheduled shift
  • Must be able and willing to “float, or be pulled” to another unit if requested to by supervisor or management due to staffing needs for quality care for our residents or may be considered a self termination if you do not follow supervisors request.

  • Sufficient mobility, and strength to move freely through the building, to assure resident safety at all times and to assist, transfer, or otherwise move residents out of danger in case of emergency.
  • Demonstrates respect for co-workers and responds to needs of residents by complying with facility policies on attendance and punctuality and dress code.
  • Able to arrive and to begin work on time and to report for duty as scheduled on all shifts, weekends, and/or holidays.
  • Working knowledge and ability to comply with facility policies and procedures for workplace safety including infection control procedure, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials.
  • Demonstrates an ability to prioritize tasks and responsibilities.
  • Demonstrates the ability to complete duties and projects within an allotted time.
  • Able to respond to change productively and to handle additional tasks and projects as assigned.
  • Able to carry out the essential functions of this job (without reasonable accommodation) without posing specific, current risk of substantial harm to health and safety of self and others.
  • Knowledge of geriatric, other specialty, general chronic care nursing principles and professional standards of nursing practice and ability to apply to resident specific circumstances. Able to identify, implement and evaluate appropriate objectives and interventions for residents.
  • Demonstrated competency in clinical skills needed for facility resident population.
  • Demonstrated competency in physical, behavioral and functional capacity assessment of long term care residents including use of the State-approved Resident Assessment Instrument.
  • Current knowledge of laws and regulations that influence provision of care and services in nursing facilities.
