We are looking for qualified Florida Licensed Practical Nurses that have the same passion and drive that we do to help our clients age safely and happily at home!

Requirements to Register as an Independently contracted Caregiver:

• Valid Driver's License or Photo ID
• Social Security Card or Employment Tax ID Number
• FLORIDA LPN
• Level 2 background screening with ACHA fingerprints
• Physical within (6) months that states you are FREE OF COMMUNICABLE DISEASES (cannot be signed by a Chiropractor)
• CPR (American Heart Association, American Red Cross, or American Safety & Health Institute)
• HIV/AIDS certificate (minimum 1-hour)
• 30 hour IV INFUSION Certification (LPN only) optional
• Previous Employment/ Contract history for the last 5 years
• Professional Liability Insurance (Strongly Suggested)
• Occupational Accident Insurance (Strongly Suggested)
