The Rehab Exchange, Inc is currently seeking a LPN (Licensed Practical Nurse) to join its growing team of nurses!
Some of the duties and responsibilities include:

Provide one-to-one care with patients in the privacy of their own homes -
Perform duties outlined in clients plan of care -
Follow-up and coordinate case with RN after evaluation -
Observe the patients physical, mental, and emotional conditions -
Maintain a professional nature within patients home -
Document patients reaction to treatment on clinical/progress notes, which are written on the day of the visit and incorporated in the patients record weekly using Kinnser (Wellksky) EMR system
Requirements

Approved LPN license in the state of Virginia

Vaccination or valid exemption
-
Current CPR license
-
TB and Chest X-ray
-
Prior Home Care experience (preferred)

Successful completion of accredited physical therapy assistant or equivalent program. Certified in the state of Virginia. Prior Kinnser EMR experience (preferred) - training will be provided Possesses and maintains current CPR certification.
