What will I do as an employee with Redstone?
As an LPN Field Nurse - Home Care/Home Health/Hospice, supporting the Redstone mission will include the following responsibilities:
- Provides nursing care to clients, in the home, in facilities, at wellness clinics, or in an Adult Day Center according to the Nurse Practice Act and under the direction of an RN. Responsible for the delivery of services to meet client needs. Follows current home health nursing professional standards of practice and agency approved protocols.
- Provides ongoing care to clients. Identifies when clients need or qualify for additional services and make referral to the appropriate manager or other community resource.
- Maintains current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines.
- Conducts Wellness clinics as assigned. Communicates needs to others as appropriate.
- In home care, supervises and monitors the performance of Homemakers and Personal Care Assistants assuring the delivery of quality health care services.
- Responsible for the implementation of the physicians' plan of treatment.
- Assist with orientation and training of new employees including field visits.
- Participate in on-call rotation per department as needed.
What do I need for this role with Redstone?
- A Licensed Practical Nurse, currently holding an unencumbered license to practice in state.
- One year of acute care experience. (Prior experience in a Medicare-certified home health or hospice agency preferred)
- Basic computer literacy with experience in Windows-based programs. (Preferred)
