Position Summary:
The LPN provides direct and/or indirect nursing care, treatment and instruction in the homes of patients referred by the Home Care department under the supervision of a qualified registered nurse. Requires excellent assessment skills, agrees and complies with "Standards of Behavior" of SCH, ERM documentation, IL license, CPR Certification, valid driver's license, and reliable transportation.
