Linkage Coordinator Lead
The Linkage Coordinator provides intensive care coordination, support and services to HIV-positive individuals who require rigorous assistance to improve their ability to access and remain in care.
Core Duties
- Leads the Linkage and Retention Unit within the HIV Program to ensure workflows, attendance, and productivity of the team are within the standards and goals set forth by the HIV Program Manager and HIV Program Director.
- Maintains a set of focused strategies, designed to improve the linkage (and ongoing engagement) to HIV primary medical care of recently diagnosed HIV-positive persons; individuals of known HIV status who have never sought medical care, and HIV-positive persons who have dropped out of care and are no longer receiving HIV medical care and treatment.
- Engages and coordinates with providers to ensure timely access to HIV treatment for newly diagnosed individuals.
- Provides counseling and rapid HIV testing as needed.
- Supports health behavior change utilizing motivational interviewing and trauma informed care practices.
- Provides intensive case management support to newly diagnosed, re-engaged, and transfer patients the first 3-6 months of receiving care at One Community Health to ensure all medical appointments and labs are fulfilled.
- Identifies patients' unmet medical needs and coordinates with medical case management, providers, and partner HIV providers(s) to ensure that standards of care are met, and any gaps or barriers are addressed.
- Identifies patients' unmet nonmedical needs or barriers to adherence and retention in care, and coordinates with other One Community Health staff (e.g., medica case managers) and community resources to address needs and barriers.
- Utilizes electronic health records, excel sheets and other data management systems to manage patients and track trends in overall HIV services performance.
- Maintains HIV linkage records to ensure that all documentation required by funders and agency Quality Management Plan is on file.
- Supports the completion of reports to funder as directed by supervisor and/or HIV Program Director.
- Communicates directly and regularly with providers on patients' medical care as appropriate.
- Screening of client eligibility and enrollment in Ryan White Program
- Serves as primary contact to individuals newly diagnosed with HIV.
Additional Duties
- Participation in community meetings and engagement activities
- Completion of Basic Counselor Skills Training (HIV Testing Counselor)
- Other duties assigned based on program and organizational need
- Completion of ADAP enrollment worker training
SUPERVISORY RESPONSIBILITIES
- Directly supervises: This position does not have supervisory responsibilities
- Oversees: This position oversees the daily operation, workflows, and attendance of staff within the Linkage and Retention Unit.
EDUCATION AND/OR EXPERIENCE AND OR LICENSE/CERTIFICATION
Required
- Bachelor's Degree in health services or related field or 3 years of relevant experience.
Preferred
- 2 Year of Experience in HIV/STD Education & Prevention
- 2 Year of Experience in HIV/AIDS service
- 1 Year of Experience as care provider for Sexual Minority Communities including but not limited to Lesbian, Bisexual, Pansexual, Gay, Transgender and Queer identified individuals preferred
- 1 Year of Experience as care provider to Communities of Color Including but not limited to Indigenous and Native, African American/Black, Afro-Latinx, Latinx, Asian and Pacific Islander preferred
