The primary function of this role is to provide coordinated care in the home to clients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family.

How you’ll make an impact as a Home Health Social Worker (MSW)

• Assess the psychosocial status of patients and families/caregivers related to the patient's illness and environment and communicate findings to the RN Case Manager and other members of the interdisciplinary team.
• Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources.
• Supervise the implementation of an individualized social work plan of care
• Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses and symptoms of the patient's illness.
• Counsel patient and family/caregivers as needed regarding stress and other identified coping difficulties.

Education and/or Experience

• Master’s degree in Social Work required.
• One (1) year experience as a Social Worker in a health care agency required.

Certifications, Licenses and Registrations

• Active and unencumbered Licensed Master Social Worker in state(s) of employment required.
