To support people with disabilities and special needs by: Provide direct nursing and medical quality assurance services to consumers living in DBCC Residential facilities
- Provides professional nursing care to all DBCC consumers in accordance with established policies and protocol of DBCC.
- Administers medication as directed by the physician, Director of Nursing or Director of Residential Services to consumers as needed throughout the Residential Program.
- Monitors medical status of consumers, reporting concerns to the DON and appropriate residential manager/director.
- Schedules medical appointments for ICF/CRCF consumers and/or any other consumers throughout the program at the request of the DON.
- Accompanies consumers to medical, dental and other appointments as required by facility protocol.
- Provides training to include but not limited to: medication administration, insulin administration, first aid, glucose meter training, and/or consumer specific protocols at the request of the DON or residential manager/director.
- Receives and follows verbal orders from physicians.
- Maintains accurate records in accordance with facility practice, and any state and federal regulations/guidelines.
- Provide quality assurance visits to evaluate, but not limited to, the facilities existing medication administration practices, documentation, storage of medications, and physician’s orders.
- Works in conjunction with management in educating families on medications and medical procedures that may be recommended.
- Work with management to ensure appropriate consent for medical interventions from the consumer and/or family member, if appropriate, is achieved in a timely manner.
- Provide assessments on consumers in and out of the hospital at the request of the DON or residential manager/director.
- Assists with the operation of the employee health office to include administration of PPDs and Vaccines to both consumers and agency staff.
- Attends psychotropic drug reviews and consumer treatment meetings as requested.
- Provide nursing assessments and documentation as needed, to the appropriate residential manager/director.
- With Physician review, performs, evaluates, and/or completes assessments/screenings such as, but not limited to, DISCUS, AIMS, and Dysphagia screenings.
- Provides and prepares documentation for continued/ongoing placement in the residential facility as appropriate i.e. utilization review, level of care.
Remains on call for 24/7 approximately every 6 weeks.
Other duties as assigned.
