The Enhanced Care Management (ECM) and Community Supports (CS) is an interdisciplinary approach to care that addresses clinical and non-clinical needs of Medi-Cal members with the most complex medical and social needs. The services provide systematic coordination of services and comprehensive care management that is community-based, interdisciplinary, high touch, and person-centered. Kings View’s vision includes coordination of all care for Medi-Cal members who receive ECM or CS, including across physical and behavioral health delivery systems. Community Supports are medically appropriate and cost-effective alternatives to traditional healthcare services and can substitute for and decrease utilization of hospitals and emergency departments. Community Supports address challenges such as unstable or unsafe housing and other social needs.
The Licensed Clinical Manager/Consultant is a supervisorial position under the direction of the Regional Director. The Licensed Clinical Manager/Consultant ensures the quality of services provided by the Care Managers that addresses the clinical and non- clinical circumstances of high- need members enrolled in Medi-Cal managed care.
ECM and CS services provide a whole-person approach to children and adults experiencing complex medical and psychosocial challenges to minimize the likelihood of preventable hospital and emergency department admissions. Services include outreach and engagement, providing person-centered intensive case-management, developing care plan, and providing linkages to medical, psychiatric, social, educational, and other services as needed.
The Licensed Clinical Manager/Consultant provides direct supervision for Care Managers and supports activities related to staff training, assessment, quality assurance and improvement, and evaluation of the services. Performance is reviewed on a continuous basis with specific goals and objectives identified throughout the year. This is a full-time, exempt position.
How will I contribute
- Providing direct supervision and staff development training for Care Managers.
- Ensuring that strategies and services are whole- person centered, linguistically, and culturally appropriate.
- Guiding Care Managers in steps to ensure the Member’s safety and providing immediate consultation and guidance for any situations needing escalation of care.
- Monitoring continuous quality improvement measures for coordination of care through documentation review, clinical consultations, peer review, and other related activities to ensure quality of services.
- Overseeing charting and documentation standards adhering to contracts with Managed Care Plans (MCPs).
- Providing supervised hours for staff accruing BBS hours (when applicable and appropriate).
- Conducting staff performance reviews Recruitment, hiring, and technical assistance for unit staff.
- Monitoring and evaluating the effectiveness and efficiency of programmatic service delivery.
- Contract compliance activities including meeting the contract objectives, documentation requirements, coordinating evaluation activities, and other performance related issues.
- Establishing and maintaining liaison with community organizations, local entities, and community stakeholders for outreach and engagement.
- Outreach activities to reach participants who are under- resourced and/ or underserved.
- Asset mapping, action plan development and implementation, and on-going evaluation for the targeted communities.
What I bring
- MA, MS or PhD in Psychology or related field
- Maintain licensure as a LMFT, LCSW LPCC, Clinical Psychologist, or psychiatrist.
- Electronic health information systems, outcomes measures, guide staff to their potential to keep at caseload appropriately sized to serve an adequate number of members and progress note timeliness. Follow all KV policies and procedures, administrative directives, all relevant county, state and federal regulations and any performance obligations contained in KV program contracts.
- Routine monitoring of electronic health systems, staff compliance with documentation requirements and that staff has adequate skills in the use of EHR.
- Ability to ensure a positive and effective working relationship with partner agencies and that Kings View’s business interests are monitored and maintained.
- Two years post licensure experience in a healthcare setting and preferred one year of supervising/directing the activities of other clinical staff in a healthcare setting.
- Assess ongoing program strengths and weaknesses then provide appropriate development and monitoring of program procedures.
- Positive leadership and clinical expertise in diagnosing, planning and treatment of MH disorders.
