Amazing Care Home Health Services is currently seeking an experienced home health intake coordinator to join our intake team located in Aurora, CO.

The Intake Coordinator is responsible for the coordination of initial home care referrals in accordance with state and federal guidelines.The Intake Coordinator supports and facilitates patient referrals from our various referral sources.

Essential Job Duties:

  • Supports and facilitates patient referrals from our various referral sources.
  • Ensures data includes the patient's primary care provider and or referring MD information as applicable; updates and adds notes on actions taken on behalf or patient within the scheduling and or record procurement process.
  • Completes the patient registration including all relevant insurance details, and any authorizations, validates coverage and or eligibility, relays to patient and or referring MD any concerns re coverage
  • Obtains medical records including progress notes, hospital reports and or discharge notes, imaging and or pathology reports, lab tests, and ensures they are uploaded to the EHR within department guidelines. Communicates with department and clinic leadership re obstacles in meeting guidelines.
  • Responsible for assuring a prompt, courteous response to patient/family and referral sources' requests for services.

Position Requirements:

Excellent communication and organizational skills. Ability to function in a high volume environment with little oversight.

Previous experience in home health

LPN or RN background preferred

Bilingual preferred but not required
