What you’ll do for us!
- Instructs patients in pre-discharge education on home infusion therapy self- administration, provides direct nursing care in patients home for the administration of home infusion therapies, and troubleshoots patient infusion concerns.
- Provides infusion and other skilled nursing care to patients and perform pre-discharge patient assessments.
- Communicates patient condition changes to supervisor, physicians, and other health care providers, as appropriate.
- Participates in the orientation of other nurses or pharmacy personnel, as requested.
- Provides leadership by working cooperatively with pharmacy staff to establish priorities and goals of patient care.
- Supports patients by phone and video conference when infusion, DME and line care troubleshooting concerns arise.
- Partners with other nurses caring for home infusion pharmacy patients to ensure they have knowledge and resources to safely care for home infusion patients.
- Supports the marketing efforts of the center by assisting the liaison team and providing educational programs to referral sources, as requested. Takes initiative to present ideas/suggestions to management.
- Performs on call as assigned, to provide 24-hour nursing availability by phone to patients.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other duties as assigned.
Who you are!
REQUIRED-
- Diploma from an accredited school of nursing.
- Current, unencumbered license to practice as a Registered Nurse specific to the state(s) the employee is assigned to work/cover for the company.
- One (1) year clinical experience involving infusion therapy, including patient assessment and venous access skills.
- Excellent peripheral IV insertion skillset.
- CPR certification.
Preferred-
- Bachelor’s degree in skilled nursing.
- One (1) year previous specialized home health experience.
- PICC training
State Specific Qualifications:
- Refer to the Licensed Practical Nurse State Matrix
Working Conditions:
- Frequent degree of travel (75-100%)
