SUMMARY
Work is performed under the general supervision of the Program Administrator with broad latitude for the use of initiative and independent judgement. Responsibilities include: Assessing, evaluating and identifying consumer needs. Developing, coordinating and monitoring individual’s plans to insure quality of life and community participation that is acceptable to the individual. Locating and coordinating services and supports to prevent and/or manage crisis. Completing documentation of all assessment, planning, monitoring and crisis activities. All activities are completed in a manner that supports the mission of LifePath Systems by putting people and families at the center of planning and service delivery.
ESSENTIAL DUTIES, RESPONSIBILITIES
- Using the Service Coordination Evaluation, the Annual Plan interview. And the ongoing relationship as the primary assessment tools, SC will identify priority outcomes and needs. All assessment activity will occur in a person/family centered manner.

- To identify, arrange, advocate and collaborate with family, friends, community resources and service providers to support progress toward consumer identified outcomes.
- To assess the effectiveness of the strategies and supports identified in the plan to support consumer progress toward outcomes. To advocate and adjust supports when progress is not satisfactory.
- To lace and coordinate services and supports to prevent and/or manage crisis situations as identified by consumers.
- To perform the “A” (Comprehensive Encounter) and “B” (Supportive Encounter) requirements of a General Revenue Service Coordinator.
- To conduct and maintain a professional relationship and behavior with consumers, stake holders, co-workers, supervisors and members of the greater community.
KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge of the human services delivery system.
- The ability to communicate effectively, both orally and in writing.
- The ability to organize and prioritize a variety of assignments while managing time and productivity.
- The ability to form and maintain effective working relationships with individuals, stakeholders, other employees and the public.
- The ability to link plans and support activities to outcomes identified by individuals.
- The ability to monitor and advocate for individuals in services.
- The ability to effectively problem solve.
- The ability to act in a professional and respectful manner at all times.
- The ability to operate a motor vehicle.
- The ability to perform basic computer functions and word processing.
- The ability to complete and maintain all agency required training and certification.
- A working knowledge of the appropriate rules, regulations, policies and procedures.
EDUCATION and/or EXPERIENCE
- Graduation from an accredited four (4) year college or university with course work in a Human Services related field such as Psychology, Social Work, Sociology, etc.
CERTIFICATES, LICENSES, REGISTRATIONS
- Must have a valid Texas Driver License and a good driving record.
- Must be able to pass criminal history and driving records checks.
- Must be able to present 3 professional or educational references.
BENEFITS
Salary: $40,000 per year
Shift: Monday - Friday Flexible 40 hour work week
