PROGRAM SUMMARY

Fresno IMPACT (Assertive Community Treatment) offers 24-hour community-based treatment for individuals who have accessed emergency response systems at a higher rate and have been diagnosed with a severe and persistent mental illness and substance use disorder. Clients referred by the County are eligible to receive therapeutic interventions, medication management and monitoring, comprehensive outpatient and community services, case management, vocational rehabilitation, peer support and supportive housing services.


JOB SUMMARY
The Housing Coordinator supervises housing specialists and all housing-related functions at the program. Locates appropriate housing opportunities for clients including short-term, emergency, transitional, and permanent options. Arranges and manages various kinds of property lease agreements. Assists clients with documentation preparation and appropriate housing placement.

  • Directly supervise housing specialists to include hiring, coaching, training, evaluating performance and imposing corrective actions.
  • Review property leases and negotiate lease terms.
  • Assist clients in obtaining necessary documents for housing placement, including completion of the VI-SPDAT and CES entry. Assist clients to complete applications; provide services out in the field, on-call and after hours as required by contract and as assigned.
  • Work closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences.
  • Attend all required community provider meetings and represent MHS HOME Fresno in a positive and professional way.
  • Conduct extensive liaison with professional members of the housing industry as well as homeless and housing providers to disseminate information about MHS HOME Fresno and to identify housing options in the community.
  • Develop and manage housing 'invoice' vouchers as outlined in program procedure for timely payment of clients' housing, and track rent payments in collaboration with the Administrative Assistant.
  • Communicate with vendors to ensure all housing payments are made in a timely manner.
  • Develop monthly reports for funding streams, and update team members on a regular basis regarding housing alternatives and changes in availability.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS
  • Must meet one of the following:
    • Minimum of 5 years of experience providing housing-related social services to clients; OR
    • AA degree in social services field and 3 years of experience providing housing-related social services to clients; OR
    • BA/BS degree and 2 years of experience providing housing-related social services to clients.
  • Fluency in English required. Fluency in Spanish, Vietnamese, Tagalog or Arabic is preferred if the position serves that population
  • Minimum of one year of supervisory experience preferred
  • Ability to communicate clearly and engage culturally and socio-economically diverse populations that inform provision to persons with psychiatric or dual diagnosis disorders
  • Demonstrated knowledge of housing needs of individuals with mental health and/or substance abuse disorders and ability to fulfill housing needs of individuals with mental and/or substance abuse disorders
  • Strong attention to detail; able to create written documentation that is legible, clear, and concise
  • Must be able to maintain the distinction between therapeutic and social relationships
  • Proficient with Microsoft Office Suite and EHR software
  • Able to react appropriately to emergency situations
  • Ability to work independently and as part of a team
  • Strong ability to prioritize work and meet deadlines
  • Valid California driver’s license and current auto insurance coverage
