Position Summary: This position will oversee the residential care staff and ensures smooth daily operations of all communities associated with the organization. This position will offer services to residents as indicated in each resident’s individual service plan within multiple communities associated with Sage Oak Assisted Living and Memory Care. In addition to residential care and operations of the home, this position provides leadership and a point of communication for all current and new employees, residents, and family members.

Furthermore, this position will provide support to residents and hands-on assistance to fellow staff. This position will assist in the development and revision of service plans and observe and report any issues or concerns surrounding residents and their care. This position will also perform certain food services, activities, environmental duties and assist with medication management.

- Assist residents with emotional health and/or problem behavior issues as indicated by individual service plan.
- Redirect and/or de-escalate residents when emotional and/or problem behavior issues occur.
- Perform other services as indicated in individualized service plans (laundry, meals, and pet care).
- Prioritizes resident care according to need and acuity.
- Maintains and ensures community is organized and operates as smooth as possible to ensure daily operations are not impeded.
- Assist with medication administration, handling, supervising, ordering, and storage as follows:
- Provides medication assistance for residents in accordance with state regulation and company policy including pouring, preparing, assisting with administration, and documentation.
- Ensures proper handling and/or administration for residents.
- Ensures accurate medication counts for narcotics and storage of medications in medication cart.
- Ensure and maintain compliance with company policies and procedures related to controlled substances and documentation as such.
- Completes medication training and/or delegation as necessary.
- Management and/or care of special medication (injectables).
- Demonstrates successful passing of medication to residents without error.
- Creates medication administration record on a monthly basis or as needed for residents.
- Maintain and audit medication administration record to ensure accuracy.
- Compares medication administration record to physician orders as needed.
- Ensures and assist with medication ordering in a timely fashion to comply with resident’s service plan
- Ensure adherence to medication in-take and documentation in medication profile record (MPR) for each resident.
- Audit, file, and maintain all pharmacy and physical orders for residents on a monthly basis or as needed.
- Adhere and assist in development and revision of individual service plans by communicating with Resident Care Coordinator and/or Management for residents needs and preference.
- Observes and reports residents’ issues or problems to management in a timely fashion.
- Continually stays in tune to each resident’s clinical needs and acuity.
- Identifies and follows appropriate meals and/or diets for residents.
- Alerts Management and/or Resident Care Coordination if unsure to complete task as indicated by service plan.
- Assist in maintaining the integrity of a resident's skin by ensuring completion of weekly skin assessments and completing documentation while on shift.
- Identifies signs and symptoms of compromised skin integrity.
- Observes and reports resident skin issues or problems to management in a timely fashion.
- Ensures completion of appropriate documentation about issued or concerns observed by direct staff.
- Adhere and assist in development and revision of resident preferences for each resident as needed.
- Collaborate with resident/family and/or Resident Care Coordinator to ensure completion of resident/family preferences as needed.
- Perform and document proper measurements of vitals on a daily basis and as need as indicated in service plan or otherwise directed.
- Notifies facility supervisor of any needed medical supplies to be purchased.
- Perform multiple proper transfer techniques as indicated by individual service plan for residents.
- Gait belt, pivot, Hoyer lift, and/or sit-to-stand.
- Observe and report if direct staff need additional training on proper transfer techniques.
- Demonstrated proper transfer techniques for direct staff.
- Communicates professionally and timely with fellow teammates, including Executive Director, Resident Care Coordinator, outside services or vendors (Home Health, Hospice, pharmacy and etc.), and family members.
- Effectively communicates with all residents, families, staff, visitors, and management in a positive, helpful, and productive manner.
- Performs food service duties, including but not limited to cooking meals, serving meals, delivering meal trays to rooms, bussing tables, cleaning dining rooms after meals, and setting tables.
- Adhere and assist with development and revision of daily and weekly menu as needed.
- Comprise or Create daily and weekly menus to be provided to direct staff.
- Ensure direct staff communicates with facility supervisor before substitutions are made on daily and weekly menus.
- Ensure daily, weekly, or monthly menu is posted and filed accordingly.
- Be present at mealtimes to prevent choking incidence for residents.
- Communicates and documents appropriately with Resident Care Coordinator and/or Management about meals refused or not consumed.
- Ensure compliance and follows plating method when preparing and presenting meals to residents.
- Collaborates and creates shopping list for each community on a weekly basis and orders or picks up orders as needed.
- Performs and ensures activities duties are completed as assigned by management and/or administration, including leading group activities, engaging in activities with individual residents, and accompanying residents to facility-sponsored activities outside the facility.
- Assist with development and revision of daily activities as needed.
- Offers, encourages, and promotes activities based on activity calendar provided.
- Offers group activity to residents, if resident refuses, offers individual activity.
- Ensures pictures are taken of activities and/or events planned and send to Management as needed.
- Post activity calendar throughout community and is filed accordingly.
- Records incidents, errors, and accidents.
- Reports resident concerns to Management and/or Resident Care Coordinator immediately regardless of whether resident requires medical treatment or services.
- Ensures and adheres to incident reporting policies and procedures.
- Collaborates with fellow staff to observe changes in resident status, needs, or preferences.
- Ensure completion of incident reports and appropriate documentation upon discovery of incident or accidents by direct staff or self.
- Track, manage, and report all incident with residents, staff, or families on a daily basis to management and/or administration as needed.

- Documentation is clear, accurate, and concise.
- Attends educational and/or mandatory facility meetings monthly to ensure fulfillment of state or provincial regulations.
- Creates, leads, and facilitates monthly in-services for all direct staff within community.
- Creates and ensures completion of comprehensive assessments for all direct staff covering continuation of educational credits.
- Maintains the ability to lift fifty (50) pounds or more, stand or walk for more than an eight (8) hour workday, and is able to push, pull, bend, twist, and reach multiple times throughout a shift.
- Reports to schedule shift on time and prepared to complete task with limited attendance or tardiness issues.
- Track, manage, and report attendance and tardiness issues to management and/or administration on weekly basis.
- Adhere to incontinence, toileting, reposition, and visual checks one residents during the duration of scheduled shift.
- Provides incontinence check on a minimum of every two (2) hours or otherwise indicated by resident service plan.
- Provides toileting assistance on a minimum of every two (2) hours or otherwise indicated by resident service plan.
- Monitors resident’s safety by visually laying eyes each resident every forty-five (45) minimum or otherwise indicated by resident service plan.
- Assist with training of all new hires and/or employees as directed.
- Assist and completes sixteen (16) hours of on the job supervised training with new staff that covers various areas (i.e. ADL’s, health conditions, prevention of accidents and injuries, first aid procedures, managing disruptive behaviors, behavior management, fall prevention, and emergency evacuation procedures.)
- Assist and ensures new direct staff understands and demonstrates knowledge about various topics (i.e. time sheets, Narc count and documentation, MARS, communications, shower logs, vital signs, assisting with medication, accident and incident reporting, sign-in and sign-out sheet, skin assessments, grievance reporting, MPR, service plan, and resident charts.)
- Ensures completion of additional on the job and/or state training in various areas (i.e. reporting abuse, neglect, or exploitation, HIPPA, infection control, conditions when to notify a manager, resident rights, emergency and evacuation procedures, cooking proficiency, ADL’s for Alzheimer’s and/or memory loss residents, managing dysfunctional behaviors, and identifying and alleviating safety risk for Alzheimer’s and/or memory loss residents.)
- Observes and reports any issues with management and/or administration during training period.
- Ensures documentation is complete and Administration and/or management is notified.
- Assist with placement and scheduling of new direct staff as needed or directed.
- Scheduling and facilitating training and re-training of employees with management and/or administration.
- Ensures tracking of all inventory or supplies purchased.
- Conduct monthly audits of all binders to ensure compliance with local, state, or federal guidelines.
- Ensures continued completion for all binders assigned to ensure adherence.
- Audits state survey binder to ensure continued compliance.
- Audits all resident charts to ensure proper filing of documentation and missing information as needed.
- Track, manage, and report any maintenance issues within or outside the home on a weekly basis or as needed.
- Actively participates in scheduling of communities.
- Participates to ensure all shifts are covered.
- Acts as last resort for coverage if no coverage is found.
- Demonstrate willingness to assist and cover shifts as needed at multiple locations or communities as directed.
- Ensures completion or fulfillment of individual community schedule to ensure coverage on a weekly basis.
- Communicate scheduling needs and/or changes to scheduling manager to ensure continuity.
- Participates to ensure all shifts are covered.
- Assist with community tours as needed or expressed by management and/or administration.
- Performs other tasks as assigned by facility supervisor and/or other members of management.
