Intake Assessor- Behavioral Health Hospital - LSW, LISW, BSW, LPC, or Registered Nurse

Full Time First Shift and Second Shift Position Available

ClearVista Health and Wellness, an affiliate of CommuniCare Health Services, is a behavioral health hospital offering treatment programs that have been developed to have a profoundly positive impact on the broad range of unique behavioral needs of our adult and aging population.

Due to census growth, ClearVista is currently seeking an Intake Assessor.

The Intake Assessor must be flexible, with excellent customer service, and be able to review clinical information and present it to the physician on call. He/She must be comfortable with entering information into Medik and Salesforce as well as verifying insurance coverage. The Intake Assessor must also understand urgency and be able to work well in stressful situations.

The Responsibilities of the Intake Assessor include:

• Receives inquiry calls and assists the caller with scheduling a face-face assessment or provides triage to the appropriate community referral based on patient need.
• Assesses or ensures assessment by a licensed, qualified mental health professional for patients who present for assessment.
• Coordinates with the physician to ensure appropriate treatment is provided either at the hospital or another appropriate provider in the community.
• Ensures appropriate screening of medical and behavioral emergency conditions.
• Completes all administrative processes of the admission for treatment including, as appropriate, the initial authorization of care with the insurance company/third-party payor.
