Position Summary
Are you a Registered Nurse (RN) looking to provide 1:1 care, manage your own schedule, and grow your skills? Accurate Home Care, and its Home Health service line, is growing and adding an RN Case Manager to its team.

Pay & Perks:

- Monday – Friday; salaried position working 40+hours/week; on-call availability to support your team by phone
- $76,000 - $85,000 per year
- Mileage Reimbursement with potential for Company Vehicle

Multiple Bonuses Including:
- Additional pay opportunities based on productivity, on-call bonus

What will I be doing?
- Complete patient visits as needed under the direction of the Director of Home Health.
- Participates in the implementation and oversight of nursing and home health aide services.
- Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
What skills do I need to bring?
- Must be licensed as a Registered Nurse (RN) in Minnesota
- Must show proof of COVID-19 vaccination or medical or religious exemption
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Must be fluent in the English language
- Valid U.S. driver’s license with minimal moving violations and access to a reliable personal vehicle
