Job Summary:
The Patient Liaison position performs duties in a collaborative effort with the Home Health division to assure good communication among all departments. Duties include but are not limited to collecting patient information on all new referrals, meeting with potential patient/families and discharge planners/social workers at referral sources.
  • Identifies appropriate referral sources and builds positive business relationships.
  • Provides education about Homecare services to referral sources and to patients.
  • Meets/exceeds established referral goals.
  • Facilitates accurate and timely completion of required documentation.
  • Serves as the primary contact for the referral source. Ensures a positive ongoing relationship between the referral source and the Homecare Agency.
  • Analyzes referral source data. Monitors the local market and the competition.
  • Recommends marking initiatives based on trend analysis and department strategy. Works with the marketing team to develop collateral.
  • Call on physicians, hospitals, skilled nursing facilities’ discharge planners and case mangers within an assigned territory to promote Homecare services.
  • Prioritize accounts in accordance with the market plan.
  • Gather and organize account-related information and provide input on key customer opportunities and service line extensions.
  • Develop and maintain relationships with potential and referral customers using approved marketing processes and answering questions or concerns in a professional manner.
  • Sells consultatively and makes recommendations to prospective referring customer and patients of the various ways Southwoods Health at Home can be of service for their needs.
  • Manage assigned budget to maximize effective use of funds and stay within allocated amount in accordance with compliance.
  • Perform analysis and improve effectiveness of sales/marketing efforts, by analysis of patient referral information.
  • Work with other marketing staff to design, create and manage marketing campaigns.
  • Research competition and new marketing opportunities.
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented.
  • Build trust, value others, communicate effectively, drive execution, foster innovation, focus on the customer, collaborate with others, solve problems creatively and demonstrate high integrity.
  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including education eligible patients about appropriate clinical research studies and technologies. Serve as an essential link between patients and all other care providers.
  • Facilitate appointments for consults and supportive services within established standards.
  • Coordinate multidisciplinary planning conferences with the use of nationally recognized care guidelines.
  • Develop patient education tools and programs. Identify barriers to the plan of care and intervene when appropriate. Follow the patient through the continuum of care.
  • Provides comfort and reassurance to patient. Maintains patient’s privacy and dignity. Be an advocate for the patient.
  • Responsible for outreach programs in an effort to establish and maintain a positive working relationship with key stakeholders (i.e. physicians, diagnostic staff, nurses, radiology staff, social staff, and consulting staff).



Requirements:
Required
  • 2+ years of experience in sales, marketing, and the healthcare industry
  • Evidence of leadership qualities
  • Exceptional communication skills
  • Organizational skills
  • Valid Driver’s License
  • Proof of current car insurance
Preferred
  • Experience or coursework in business and management
  • Bachelor’s degree and/or RN/LPN with active license in Ohio
  • Current AHA BLS certification (if RN or LPN)
  • Extensive experience in all aspects of relationship development and management
  • Prior experience in healthcare marketing
  • Passion for direct selling, accountability for driving results and an entrepreneurial spirit are all vital characteristics
  • Computer literacy
