  • The Natick Walpole VNA has an exciting full-time opportunity for a Community Outreach / Marketing Liaison. We are offering competitive salary and excellent benefits!

The Community Outreach Liaison interacts with hospital, nursing home and assisted living discharge planners and case managers weekly. Meets with clients and families at designated facilities and/or MD Offices and in the community, attends all business networking events, delivers presentations, participates in local provider meetings and insures agency goals are met. The liaison promotes and encourages the referral of home healthcare services and the transition of patient’s home. Responsible for insuring complete, accurate patient information from referral sources to intake. Additional responsibilities include assistance in obtaining documentation for orders, Face to Face documents and addendums, and correction of ADR’s. The liaison works to develop and implement the agency’s marketing and business development plan under the supervision of the Director of Marketing and Business Development.

Responsibilities and Duties:

  • Generates referrals/admissions to meet monthly goals
  • Establishes ongoing productive relationships with referral sources
  • Establishes the patient’s eligibility for home care
  • Communicates with the patients and families of patients about services and resources available
  • Works with the Director of Marketing and Business Development to identify key accounts and interact with them (hospitals/SNFs/MD offices) on a regular basis
  • Assesses patients upon admission to the hospital or medical facility as requested
  • Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the discharge process and back to the home environment
  • Develops relationships in the community on behalf of the agency in accordance with the agency’s business development plan.
  • Represents interests of the agency at professional organization meetings as needed
  • Assists in obtaining documentation related to F2F’s, ADR’s, addendums and physician orders.
  • Assists Intake Department as needed

Skills/Competencies:

  • Maintains a professional and supportive work environment
  • Working knowledge of computer and ability to learn home care software package
  • Working knowledge of home care reimbursement
  • Working knowledge of OASIS document

Requirements/Qualifications:

  • Clinical background or familiarity with medical terminology
  • Recent liaison, discharge planning or case management experience
  • Evidence of achieving referral goals within the market
  • Excellent planning, organizational, and presentation skills
  • Ability to travel to local facilities and physician offices as needed.
  • Knowledge of marketing and business development
  • Strong written and oral communication skills
  • Must demonstrate ability to work in a diverse, fast-paced, team-oriented environment
