GENERAL SUMMARY AND PURPOSE:

The Health Records Abstractor facilitates the transfer of patient health information from paper and/or current electronic health records to the State TSE database and GWTG and other performance improvement databases. Identifies key data elements in the medical record per established protocol and accurately inputs data into the electronic health record.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

High School Diploma or equivalent. Must meet one of the following qualifications:

  • Associate degree in Health Information Management
  • Coding and Data Abstractor certificate
  • Two years’ experience managing data/metrics
  • Two years’ clinical experience in hospital or clinic preferred as a CNA, MA, Scrub Tech, CST, ED Tech or similar

ESSENTIAL FUNCTIONS:

Meets Health System's Core Values and exhibits ICARE standards including interpersonal communication and professional conduct expectations.

Abstracts discrete medical information from hardcopy (paper charts) / electronic patient medical records and accurately inputs it into the TSE database, GWTG, and other performance improvement databases. Follows procedures to ensure appropriate review and accuracy of medical information. Runs reports as applicable for completion of work.

Scans medical record documents from paper charts or exports/imports documents from existing EHRs into new EHR system and files them correctly. Assists with training new abstractors when applicable. Reviews and audits patient records for quality assurance upon request. Protects patients' rights by maintaining confidentiality of personal information and follows guidelines for HIPAA's regulations.

Ability to use knowledge and skills to scan, import, and enter data in EHR system or perform other tasks necessary to support Stroke Teamwork processes as needed. Consistently performs at or above established productivity and quality standards. Creates an accurate and complete daily entry in the access tracking database.

Communicates effectively and respectfully in interactions with Stroke CNS and Stroke Coordinator at both SARMC and Nampa.

Maintains attendance requirements as per policy.
