ACHC is a Federally Qualified Health Center and licensed primary care clinic. We provide medical and dental care, with additional specialists in Chiropractic, Internal Medicine, Neurology, Obstetrics and Gynecology, Pediatrics, Psychology, Podiatry and Optometry. ACHC Clinics are located across Fresno, Kings and Tulare counties.

SUMMARY*

The Health Educator works closely with our medical team and patients to promote wellness and helping them understand their conditions and/or options. In addition, the position of Health Educator will be responsible for running health events, health classes and attend community coalitions/meetings. Traveling for work is to be expected.

RESPONSIBILITES

  • Available onsite for immediate referrals. May also schedule appointments at clinic locations.
  • Individual health education counseling performed as needed.
  • Knowledgeable of Family Planning & Birth control methods.
  • Group classes performed monthly on varying topics (Weight Management, Diabetes, Hypertension, Asthma, Agricultural Work Safety, Pesticide safety, and other health related topics).
  • Coordinate class schedules and events in conjunction with the Health Education Manager.
  • Assist in the development and evaluation of curricula to enhance health education provided throughout Aria Community Health Center and community outreach.
  • Distribute, organize, and monitor educational materials and administer assessments.
  • Review health education materials for content, layout, cultural-sensitivity, and linguistic appropriateness.
  • Identify and/or develop additional educational resources, as needed, to support and reinforce health education messages.
  • Provide health education training and support for other (non-clinical) department staff.
  • Interface and collaborate with diverse group of health care professionals and outside community resources to coordinate referrals/linkages and patient care.
  • Assist in the development of the health education work plan.
  • Document services and interactions on EMR where applicable.
  • Lead health education initiatives for clinic/department sites.
  • Drive between sites as needed.
  • May be required to work evenings and weekend events.
  • Responsible for performing other duties as assigned.
  • Present at community health events throughout the year.

JOB REQUIREMENTS

  • Regular attendance and punctuality.
  • Willing and able to work a flexible and sometimes variable schedule.
  • Must possess excellent communication and interpersonal skills.
  • Travel between clinic locations.
  • Experienced in using the suite of Microsoft Office products (excel, word, publisher, power point) and capable of learning to utilize the electronic health record (EHR) NextGen.
  • The ability to speak, write and read in Spanish is preferred.
  • CHES or planning on completing the CHES within first year of hire.
  • The Health Educator receives training in family planning, chronic illnesses, smoking cessation, and lactation/breastfeeding education.

PHYSICAL REQUIREMENTS

Hearing: Adequate to perform job duties in person and over the telephone.
Speaking: Must be able to communicate clearly to patients/employees in person and over the telephone.
Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
Other: Requires occasional lifting and carrying items weighing up to 20 pounds unassisted. Requires frequent bending, reaching, and repetitive hand movements (specifically keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
