Position Description

POSITION DESCRIPTION

Natividad is currently seeking to fill one (1) full time grant funded Health Education Assistant for approximately 3 years. Under general supervision, to assist and participate in all phases of the public health education program; research, prepare and present health educational materials and oral presentations to promote public health within the community; and perform other duties as required.

Examples of Duties

THE SIGNIFICANT DUTIES OF THE POSITION ARE AS FOLLOWS:

  • Conduct outreach and identification of patients in need of assessment for Medication Assisted Treatment (MAT).
  • Conducts community surveys and research to identify health related needs; assess program effectiveness and determine ways to improve services.
  • Introduce patients to MAT programs and services and conduct brief initial assessments.
  • Facilitate appointments at outpatient MAT clinics for patients.
  • Serve as a resource and coach for their clients and assist them to access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities.
  • Coordinate and assist with educational sessions for medical providers about substance use disorders and treatments.
  • Assists medical providers in planning and participating in evaluation of educational/counseling needs, presenting health education programs and demonstrations.
  • Develops and maintains communications with various community, professional, and government organizations that provide services to clients and to promote health education programs.
  • Responds to public inquiries concerning MAT activities and functions as a resource of information and performs referral duties regarding related health and social services available in the community.

MINIMUM QUALIFICATIONS

A combination of experience, education and/or training which substantially demonstrates the following knowledge, skills and abilities:

Working knowledge of:
  • Basic personal computer usage
Some knowledge of:
  • Concepts of public health services
  • Principles, methods and practices of health education
  • Group dynamics
  • Medical terminology
  • The principles, methods and techniques used in counseling
  • Basic math and basic statistical concepts
  • Evaluation and treatment of substance use disorders
Skill and ability to:
  • Compile, organize, analyze, summarize data, and draw logical conclusions
  • Write effectively in order to produce educational materials such as exhibits, posters and pamphlets, letters, keep accurate records, etc.
  • Operate a personal computer and utilize software for the purposes of data management and word processing
  • Communicate effectively to groups and individuals from a variety of socioeconomic and cultural backgrounds
  • Develops and maintains effective working relationships with those contacted in the course of work; provide excellent customer service
EXAMPLES OF EXPERIENCE/EDUCATION/TRAINING:

Any combination of training, education and/or experience which provides the knowledge, skills and abilities and required conditions of employment listed above is qualifying. An example of a way these requirements might be acquired is:
Experience:
  • Two years experience as a community worker or a related position which included outreach education experience within a public health or voluntary non-profit health related agency, preferably dealing with “at risk” populations.
Education:
  • Completion of course work leading to a bachelor’s degree in public/community health education, education, behavioral or life sciences, sociology, biology or psychology.
Highly Desired:
  • Case management experience
  • Certification to perform the ASAM Criteria Placement Assessment.
  • Experience working successfully with issues of substance use, mental health, criminal background and other potential barriers to economic self-sufficiency.
  • Registration or Certification from a recognized certifying organization, agency, or Board evidencing training and expertise in areas appropriate to SUD services (CCAPP, CADTP, CADDE).

CONDITIONS OF EMPLOYMENT
  • Natividad requires that all incumbents pass a pre-employment physical/medical assessment.
  • Natividad will conduct a thorough background and reference check process which includes a Department of Justice fingerprint check.
  • Employees who drive on County business to carry out job related duties must possess a valid CA Driver License for the class vehicle driven and clean driving record.
  • Employees must have and show their original Social Security Card and a valid CA Driver License or CA State ID prior to the first day of work.
  • Incumbents may be required to work all shifts, including weekends and holidays;
  • Incumbents may be required to work with potentially hazardous and infectious substances.
