Group Home Program Coordinator Job at Fraser
Fraser is seeking passionate, flexible, and dedicated individuals to provide service coordination for clients in two of our group homes in Brooklyn Park and Maple Grove. Applicants with strong communication and supervisory skills are strongly preferred.
Responsibilities:
As a Program Coordinator, you will:
- Develop and assure documentation of outcome-based recommendations, Coordinated Service and Support Plan Addendums, and individual behavior/therapeutic plans for each client served
- Assure accurate implementation of client care plans
- Provide supervision and leadership to direct care staff
- Comply with all regulations, policies, day-to-day operations, and budgetary parameters of Fraser's Community Living division
- Oversee all operations of the site, including staffing, budgets, client medications, and documentation
- Regularly coordinate and facilitate person-centered planning sessions as determined by the interdisciplinary team for each individual served
- Coordinate weekly meal planning, grocery shopping, and meal preparation for each individual
- Provide instruction and assistance to direct support staff, including direct observation, performance evaluation and recommendations for corrective action, if needed
- Assess customer satisfaction, service delivery and regulatory compliance to determine opportunities for improvement

Available Locations and Schedules:
- This person will oversee the operations at two of our group homes, one in Edina and the other in Eden Prairie.
- The regular schedule for this position is Monday - Friday, during standard business hours (approx. 8am-5pm).
- Coordinators also participate in an on-call rotation, which occurs approximately every 10 weeks.
Requirements:
Requirements for the Designated Coordinator position are a combination of education and experience, as outlined by MN Statute 245D. Please read carefully before applying.
Candidates must qualify based on one of the following:
- BA/BS in Human Services or related field with 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older
- Associate degree in Human Services or related field with 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older
- A minimum of 50 hours of education and training related to human services and disabilities with four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older, under the supervision of an individual qualifying as a Coordinator under 245D.
Additional Requirements:
- Valid drivers license with acceptable record
- Ability to meet DHS standards on criminal background study
