POSITION SUMMARY
The CCHS geriatrician provides professional comprehensive medical services for older adult ambulatory patients. The geriatrician will see primary care patients and geriatrics referrals from other primary care providers. He or she also advises CCHS leadership on program development for older adults. Incumbents will provide professional medical services for outpatients on a full-time basis as described in their contracts.
KEY RESPONSIBILITIES
1. Provides patient care to older adult patients as a part of a patient-centered medical home care team. Fosters and encourages an attitude of teamwork.
2. With support from team, addresses areas of care that are important for aging patients including activities of daily living, medication side effects and interactions, and advance care planning.
3. Participates in organizational mission to provide distinctively Christian healthcare.
4. Collaborates with and/or supervises nurse practitioners or physician assistants, as appropriate, in providing patient care; may assist in the education training of medical students and residents.
5. Collaborates across departments including dental, behavioral health and spiritual health.
6. Attends staff and medical provider meetings and maintains a level of involvement with the health center committees, as needed, to ensure quality patient care.
7. Participates in continuing medical education, including Life Support Training, to maintain the skills necessary to perform physician duties and as required by applicable medical boards and the State.
8. Geriatric physicians are to participate in peer assessment, performance evaluations/improvement data, and other activities to evaluate and advance quality improvement measures
9. Maintains own physical health required to perform physician duties; maintains current immunizations and up-to-date PPD.
10. Documents all patient encounters in a timely manner in hospital and/or clinic health record.
11. May perform other duties as assigned by the Chair of Medicine.
Requirements:
POSITION REQUIREMENTS
Education: Doctor of medicine or osteopathy degree from an accredited institution of medicine or osteopathy
Experience: Completion of residency program with Certification of Specialty Training
Licenses or Certifications: Board Certification (or eligible) in area of specialty training and maintenance of certification; State licensure to practice medicine in the state of Tennessee.
