Case Manager – Residential Treatment Facility

Emerald Isle Health and Recovery is looking for a hardworking and dedicated Case Manager looking to demonstrate a strong commitment to providing the best possible experience for our guests and associates.

Role and Responsibilities

• Attends all clinical and treatment plan staff meetings.
• Completes all necessary paperwork within three days of clients’ admission date.
• Meets with all clients in Detox and Residential Treatment by the conclusion of the third day of the client’s attendance to discuss initial discharge planning.
• Provides client with a detailed orientation, including a walk-through of the client binder.
• Maintains records involving client discharge/aftercare plans, clinical and medical progress in treatment, individual needs and financial demographics.
• Communicates client information with clinical staff.
• Works with the clinical team to plan family communications and family sessions.
• Responsible for family communications regarding client progress as needed or directed by Executive Director or Clinical Lead.
• Develops appropriate referral options for clients’ discharge with communication and coordination with clinical and admissions personnel.
• Meets with clients prior to discharge to discuss discharge plan, plan for employment, attending AA meetings, maintaining a sponsor, etc.
• Contacts past clients to follow up with how they are doing since their discharge from Liberty Bay.
• Adheres to all confidentiality laws and maintains ethical boundaries with clients.
• Assists clients with paying bills or taking care of outside obligations.
• Assists with client transportation as needed.
• Helps with any other activities or duties that are required.

Qualifications and Education Requirements

• High School Diploma or equivalent.
• 2+ years' experience in care coordination or clinical practices.
• Current CPR, First Aid and Bloodborne Pathogens certifications.
• Satisfactory drug screen and criminal background check.

Preferred Skills

• Substance abuse treatment history.
• Excellent written and verbal communication skills.
• Strong organization and coordinating skills.
• EHR systems experience.
