Company: Regional Mental Health Center, is a progressive, not-for-profit, comprehensive community mental health center, with over a dozen locations, located within 45-minutes southeast of downtown Chicago in Northwest Indiana. Regional is part of the system of private non-profit mental health centers that have successfully served Indiana for over 40 years. Regional Mental Health Center offers a comprehensive range of professional services to meet the mental health and addiction treatment needs of adults, children, and families. Services can vary from parenting or stress management classes for the general public, to crisis intervention services or hospitalization for an acute psychiatric episode. As an organization that specializes in providing behavioral health programs, Regional Mental Health Center also provides such services as counseling for marriage, child-related, and aging concerns, as well as treatment for addiction problems and serious mental illness. We provide individual, couple, group and family therapy, and psychopharmacology services at multiple locations. The Strawhun Center in Merrillville, Indiana has an inpatient program, which provides acute hospital care for mild to serious psychiatric disorders. Additionally, addictions programs are provided for adults needing residential care or intensive day treatment at both the East Chicago and Merrillville, Indiana locations. Outpatient psychotherapy groups for substance abusers are also provided at multiple locations.

Position Purpose:

Responsibilities would include providing case management and ADL services to SMI, CA and SED clients and their families. These services are directed toward: providing the skills and supports necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case managers provide assessment, service planning, coordination and linkage and monitoring functions.

The Case Manager (CM) interacts with a variety of treatment providers to coordinate services a needed. The Case Manager (CM) provides linkage to community in order to ensure client’s medical/health, educational, vocational, and housing needs, as well as maintaining benefits. The CM also provides other skills trainings services necessary for successful independent living, such as activities of daily living, arranging transportation, or medical appointments. If a client is on psychotropic medications, the CM works to ensure that the client is obtaining and taking medication as prescribed. They may also coordinate with family members and significant others. The CM is responsible for completing documentation and assessments as needed. The CM will also be responsible for maintaining payee responsibilities for persons. The individual must attend regular meetings and other duties as assigned.

Requirements:

· Bachelor’s degree in psychology, social work, or related clinical field

· 1 year of experience with the target population
