Your Goal:

To provide outreach and provides education for enrollment in Family Case Management for eligible families. In this role you will be working with expecting mothers and families in making sure they have the support they need and access to community and state resources.

How You'll Help Patients:

• Provide adequate education for enrollment of Family Case Management eligible families.
• Timely management of the Illinois DHS Case finding list.
• Assist Case Managers with documentation in Cornerstone and accurately documents client activity in Cornerstone.
• Provide outreach and completes enrollment of FCM eligible families by contacting potential clients with phone calls and letters.
• Participates in community outreach events and gathers additional resources for clients.
• Performs Ages and Stages Questionnaires with applicable clients as needed.

• Associates degree required.
