Social Service Specialist 2

Do you have the compassion and motivation to work with and protect seniors and people with disabilities? Join our team and be part of our hard-working team of professionals at the Everett or Renton Home and Community Services Office.

In this role, you will support the ALTSA mission to transform lives by promoting choice, independence, and safety through innovative services. This role supports Social Service Staff to help them implement a plan of care for a population of high-risk, elderly/frail, and vulnerable persons aged 18 years and older, who are medically and financially dependent and in need of the full range of Home and Community Services (HCS) programs and services. This role supports Social Service Staff to help maintain client safety and wellness in the community setting and the client's setting of choice by supporting HCS Social Service Specialist 3's who determine a client's functional eligibility for LTSS services and support.

Snohomish County employees are eligible to receive an Orca Card for transportation needs. King County employees receive 5% premium pay in addition to the advertised salary and are eligible to receive a free ORCA card for transportation use.

Some of what you will do:

  • Upon referral from SSS3's, or Supervisor, liaison with Consumer Direct Washington on any individual provider issue and monitor the progress in hiring these providers
  • Monitor the progress of home care agency referrals and residential facility searches, address any barriers, and confirm the start of services
  • Upon referral from SSS’s assist in connecting clients with Durable and Specialized Medical Equipment and Environmental Modifications
  • Connect with clients who have recently been discharged from Acute Care Hospitals to determine their need for services.
  • Conduct home visits to monitor service plan and obtain signatures on documents
  • Monitor the progress of referrals, address any barriers, and confirm the start of services
  • Document all activities in SERs in CARE
We are looking for professionals who demonstrate:
  • Ability to communicate and comprehend through oral and written forms of communication
  • Coordinating multiple tasks in various stages of completion
  • Proficiency in using a computer and various computer programs
  • Maintaining strict confidentiality of all client data
  • Handling intense and high-pressure situations
  • Knowledge of community resources for the elderly, disabled adults, and caregivers

Who should apply?
Professionals who have one of the following:
  • Twelve months of experience as a Social Service Specialist 1 and completion of this agency's Social Service Specialist training program
  • A Bachelor's degree in social services, human services, behavioral sciences, or an allied field, and two years of paid social service experience
  • A Master's degree in social services, human services, behavioral sciences, or an allied field, and one year of paid social service experience

Please Note: A two-year Master's degree in one of the above fields that included a practicum will be substituted for one year of paid social service experience.
