Mission Health Services is a non-profit organization, dedicated to creating a world of change for our residents and care partners! Summit Vista is expanding its retirement facility to provide care to those individuals with ailments from ageing allowing the organization to reach and provide care to even more Utahn’s in the Salt Lake area. Meadow Peak is a proud partner of Summit Vista. As Utah’s first Life Plan Community employees aim to provide basic daily care in a safe and secure environment. The Director of Social Services will help fulfill the care requirements of our patients in a warm, hotel style environment. The nurse will support the Administrator with management of the Social needs of the residents.

This position provides a unique flexibility to a nurse who would like to both lead and manage around a personal schedule.

Responsibilities:

• Initial Social Services Assessments
• Resident and Family case management
• Discharge planning
• Lead and coordinate with the Dept Heads
• Documentation
• Other duties as assigned

Requirements

• Licensed Social Worker or Experience as a Resident Care Coordinator
• Strong desire to care for elderly
• Able to pass background screening
